Office Financial Policy
Waterfront Endodontics is committed to providing you with the best possible care. We have created a financial policy to help make it affordable for you to receive the necessary dental care to attain a healthy smile. Our financial policy explains how our specialty office handles payments for dental services. Fees quoted will be based on the specific tooth involved, the type of treatment necessary, and the extent of the treatment.
The following options are available to patients without dental insurance:
- Our consultation fee is due at the time of service. Our current maximum fee total for a consultation is $286.00.
- Payment in full upon completion of treatment will receive a 5% courtesy.
- Three equal monthly payments, with the first estimated payment due within 30 days of finalizing treatment.
We accept checks, cash, Visa, Discover, MasterCard or American Express as forms of payment. We also offer interest-free financing plans through Advance Care (see details below).
If these options do not work within your dental budget, please speak with our office coordinator to request a custom written financial policy. All written financial policies will include the $286.00 consultation fee prior to beginning treatment.
Patients with Dental Insurance
A $250.00 deposit will be required before beginning dental treatment unless otherwise discussed.
If you provide our office with your dental insurance information at least 2 business days before your appointment, our staff will obtain an estimation and verification of benefits. If given on the day of your appointment, we will obtain this information within two business days for an estimation and verification of benefits. You may scan/fax your card front and back or provide us with the following information from your card.
- Patient Name
- Patient Date of Birth
- Subscriber Name (if different from patient)
- Subscriber Date of Birth
- Member/Employee ID
- Group Number
- Insurance Company Name
- Insurance Company Address (if on card)
- Insurance Company Phone
As a courtesy, we will submit your insurance claim at the completion of your treatment. Please keep in mind, insurance companies routinely indicate that coverage verification does not guarantee payment and quoted amounts are estimates only.
We will bill your insurance, and upon receipt of their payment, we will send a statement with your remaining balance.
Advance Care offers revolving credit products provided by leading banks. Interest-free periods are available and include 6-month and 12-month programs for those who qualify. If a check is returned for insufficient funds we will present the check a second time on your behalf. A $25 charge will be added to your account to cover the bank fees we incur and any future payments for services must be paid by cash, credit card, cashier's check or money order.
All fees incurred by Waterfront Endodontics to collect an outstanding balance will be paid by the patient whose failure to pay his/her account caused these fees to be incurred. Submission to treatment implies consent as outlined in this service agreement. Any courtesy treatment will be removed from your account and the full balance will be submitted to collections.
